Welcome to Revive Med Beauty (“Revive Med Beauty,” “Company,” “we,” “our,” or “us”). Revive Med Beauty is a brand operated by Matthew Gerald Dubes, doing business as Revive Med Beauty.

Revive Med Beauty provides consultation request and coordination services for aesthetic consultations offered by partnered medical spas and providers (“Providers”). Our Services may include:

Collecting consultation requests through our website forms;

Contacting you by phone call and/or SMS text to follow up on your request;

Coordinating scheduling with a Provider;

Sending a secure deposit payment link (where applicable) to reserve or confirm a consultation time.

We do not provide medical care, medical advice, diagnoses, or treatments. Any medical services are provided solely by the Provider.

2. Eligibility

You must be at least 18 years old and legally capable of entering into a binding agreement to use the Services.

3. User Responsibilities

You agree to:

Provide accurate and complete information when submitting a consultation request;

Respond promptly to communications related to your request when possible;

Treat Providers and their staff respectfully;

Comply with all applicable laws and regulations.

You understand that submitting a request does not guarantee appointment availability.

4. Deposits, Payments, and Refunds

a. Deposits

In some cases, a refundable deposit may be required to reserve or confirm a consultation appointment time. If a deposit is required, we may send you a secure payment link (for example, via Stripe), or you may choose to provide payment information by phone for manual processing.

b. Payment Processing

Payments are processed through third-party payment processors (such as Stripe). We do not store full card numbers.

c. Refunds

If you request a refund of an eligible deposit, you must contact us at [email protected]. Refund eligibility and timing may vary depending on the circumstances and whether the appointment was confirmed or completed.

5. Communications (Calls & SMS Text Messages)

If you submit your phone number through our Services and opt in, you agree that we may contact you via phone call and/or SMS text message for transactional purposes, including:

Follow-up regarding your consultation request;

Appointment coordination;

Sending a secure deposit payment link;

Confirmations and reminders.

Message frequency varies. Msg & data rates may apply.

You may opt out of SMS messages at any time by replying STOP. Reply HELP for help.

6. Privacy Policy

Your use of the Services is also governed by our Privacy Policy, which explains how we collect, use, and share information.

7. Use of Website and Content

All content on our website, including text, graphics, branding, and design elements, is owned by or licensed to Revive Med Beauty and may not be copied, reproduced, distributed, or exploited without permission.

You agree not to use the Services for any unlawful purpose or in a manner that could harm the Company, Providers, or other users.

8. Limitation of Liability

To the fullest extent permitted by law, Revive Med Beauty, Matthew Gerald Dubes (DBA Revive Med Beauty) and their affiliates, owners, employees, agents, and service providers will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to:

Your use of or inability to use the Services;

Provider services, medical outcomes, or scheduling decisions;

Communications interruptions or delays;

Third-party payment processing.

To the extent permitted by law, our total liability for any claim related to the Services will not exceed the amount you paid to us in deposits (if any) in the three (3) months preceding the event giving rise to the claim.

9. Indemnification

You agree to indemnify and hold harmless Revive Med Beauty and Matthew Gerald Dubes (DBA Revive Med Beauty) from any claims, liabilities, damages, losses, or expenses (including reasonable attorneys’ fees) arising out of or related to:

Your use of the Services;

Your violation of these Terms;

Your interactions with Providers.

10. Modifications to the Terms

We may update these Terms from time to time. The updated version will be posted with a new effective date. Continued use of the Services after changes are posted constitutes acceptance.

11. Governing Law

These Terms are governed by the laws of the State of California, without regard to conflict-of-laws principles.
